How are your donors screened?
1. Donors are initially pre-screened to ensure they meet the Donor Criteria of our program. We follow FDA and ASRM.
2. They are then asked to fill out an extensive donor profile, which includes their detailed social, medical and health history.
3. We interview the Donor to get to know her and discuss her motivations for wanting to donate. Interviews are completed in person, over the phone and via Skype webcam.
4. Once a donor is chosen and contracted, we proceed with psychological and medical evaluations and testing.

How do I secure a donor through your agency?
- First, review our Web site and contact us to request the password to access our secure donor database.
- Once you notify us that you have selected a donor, we will confirm her availability and we will provide you with a Letter of Intent which will outline the expected costs, provide a schedule of payments, request a deposit to begin work on your behalf and put an official hold on your
selected Donor.
- Upon receipt of your deposit, we will draft and issue a Services Agreement for your review and signature and a temporary hold will be placed on your selected donor.
- When we receive your signed contract and next payment, your Donor will officially be secured.

Are your donors always available?
All of our donors are available to view in our online donor database. We mark a donor's profile as "in cycle", "on hold" or "not available" in order to help with your selection process. If a donor is "in cycle" it usually takes 2-3 months to be approved to donate again. A donor is placed "on hold" when a contract is in the process of being issued and signed. A donor will only be confirmed and placed "in cycle" once a signed contract and all necessary payments have been received. If you are interested in someone who is "on hold", please contact us for more information regarding their status and availability.

Why should we choose to work with your agency?
- We work closely with you, your Donor and Clinic Coordinator. This allows us to give you the personal service that is so necessary during this process.
- We have personal experience with infertility and the egg donation procedure. This helps us to understand your needs better than someone who has not been there.
- We are a full-service agency. We screen all donors, coordinate psychological evaluations, medical testing and legal consultations for donors. We arrange travel for donors, secure Accident and Medical Procedure Complication Insurance and we pay everyone contracted on your behalf.
- We will execute and coordinate the agreement between Heartfelt Egg Donation, LLC, you and the Donor. Our contracts are in compliance with current egg donation laws and regulations and will be specific to your donation arrangement.
- We are flexible and meet the needs of our clients with unusual schedules. We will do our best to arrange our schedules to work with your needs.

What type of egg donation agreements do you arrange?
- Anonymous Agreements - Confidential-identifying information will not be provided to either the Intended Parents or the Donor. No personal meetings will be coordinated.
- Semi-open Agreements - Confidential-identifying information will not be provided to either the Intended Parents or the Donor. Both parties may agree to meet in a personal setting with Heartfelt Egg Donation present as the facilitator.
- Open Agreements - Confidential-identifying information for both the Intended Parents and Donor will be released. A personal meeting may also occur between Intended Parents, Donor and Heartfelt Egg Donation present as the facilitator. Both parties must agree for an open donation to occur.

Do you have any restrictions regarding the Intended Parents you work with?
We do not discriminate on the basis of personal lifestyle choice, household situation or medical need.

Do we have to undergo psychological screening?
We do not require Intended Parents to undergo psychological screening to participate in our program. More than likely your physician will require you to meet with a psychologist in order to move forward with an egg donation cycle.

Is Heartfelt Egg Donation a licensed egg donation agency?
Heartfelt Egg Donation, LLC is licensed to conduct business. There are no formal licensing requirements for an Egg Donation Agency in Nevada. Heartfelt Egg Donation, LLC is licensed as a registered Tissue Bank in the State of New York. We follow FDA guidelines for Tissue Donation.

How much are your egg donors compensated?
Hearfelt Egg Donation has signed an agreement with the Society for Assisted Reproductive Technology (SART) that states that we will abide by the American Society for Reproductive Medicine (ASRM) Ethics Committee Guidelines governing the payment of egg donors. The guidelines pertaining to appropriate donor compensation specifically state: "Total payments to donors in excess of $5,000 require justification and sums above $10,000 are not appropriate." Donors will not be compensated over $10,000 under any circumstance.
